  • We take pride in the time and specialized care we provide each patient.

    Insurance plans often severely limit the time a practitioner can spend with a patient and sometimes the type of care; therefore, Cooper Concierge Care, LLC does not participate with in-network plans.

    We are not contracted with any medical insurance companies and are therefore considered “out-of-network.”

    Payment is due at the time of service. We accept most major credit cards

    We do not bill insurance.

    The amount of reimbursement will depend on your specific insurance plan and deductible. We do not guarantee reimbursement.

    We encouraged you to contact your insurance plan to determine if you will be reimbursed for any of your services.

    However, you can utilize your HSA/FSA account.

    We do not accept Medicare, Medicaid, or Tricare. You may NOT file a claim with any of these government health place as Katelin Cooper, APRN has opted-out of these plans.

  • Yes, we are contracted with Quest Diagnostics.

    They have phlebotomy centers available and will be able to bill your labs through your medical insurance provider.

  • We currently offer in-person appointments in our Bentonville, AR office and Telehealth appointments for individuals living in Arkansas and Washington

  • Yes. Concierge medicine is not a substitute for insurance. Insurance is necessary to cover costs related to blood work, imaging, specialist appointments, medications, and hospitalizations. However, you may choose a high-deductible insurance plan with a health savings account.
